耳介・外耳道の疾患

🧠 全体像：外耳疾患は「形成異常・外傷・閉塞・炎症・腫瘍」に整理できる。耳珠圧痛tragal tenderness 耳珠圧痛（tragal tenderness）tragal tenderness（耳珠圧痛） と正常鼓膜 normal tympanic membrane 正常鼓膜（normal tympanic membrane） normal tympanic membrane（正常鼓膜） は外耳炎 otitis externa 外耳炎（otitis externa） otitis externa（外耳炎） を示唆し、高齢糖尿病患者の治療抵抗性耳痛 otalgia耳痛（otalgia） otalgia（耳痛）・肉芽granulation 肉芽（granulation）granulation（肉芽） は壊死性外耳炎necrotizing otitis externa壊死性外耳炎（necrotizing otitis externa）necrotizing otitis externa（壊死性外耳炎）として緊急対応する。

先天異常

聴力と中耳・内耳形成を画像評価し、耳介形成術、外耳道形成external auditory canal reconstruction 外耳道形成（external auditory canal reconstruction）external auditory canal reconstruction（外耳道形成） 術、骨導補聴機器bone-conduction hearing device 骨導補聴機器（bone-conduction hearing device）bone-conduction hearing device（骨導補聴機器） などを個別に選択する。

外耳外傷

創洗浄は清潔な水または生理食塩水 saline 生理食塩水（saline） saline（生理食塩水） による十分な洗浄とデブリードマン debridement デブリードマン（debridement） debridement（デブリードマン） を基本とし、組織障害を起こし得る高濃度過酸化水素 hydrogen peroxide 過酸化水素（hydrogen peroxide） hydrogen peroxide（過酸化水素） などをルーチンに創内へ用いない。熱傷に氷や家庭用油脂を塗布せず、凍傷frostbite 凍傷（frostbite）frostbite（凍傷） 部を擦らない。

耳垢栓塞

耳垢Cerumen 耳垢（Cerumen）Cerumen（耳垢） は耳垢腺 ceruminous gland耳垢腺（ceruminous gland） ceruminous gland（耳垢腺）・毛包脂腺 sebaceous gland 脂腺（sebaceous gland） sebaceous gland（脂腺） 分泌物、脱落上皮desquamated epithelium脱落上皮（desquamated epithelium）desquamated epithelium（脱落上皮）、粉塵dust 粉塵（dust）dust（粉塵） の混合物である。狭い・多毛hypertrichosis (excess hair growth) 多毛（hypertrichosis (excess hair growth)）hypertrichosis (excess hair growth)（多毛） な外耳道、補聴器、綿棒cotton swab綿棒（cotton swab）cotton swab（綿棒）、骨腫で自浄 self-cleaning 自浄（self-cleaning） self-cleaning（自浄） が妨げられる。

• 鼓膜穿孔tympanic membrane perforation鼓膜穿孔（tympanic membrane perforation）tympanic membrane perforation（鼓膜穿孔）、チューブtubeチューブ（tube）tube（チューブ）、耳手術歴history of ear surgery耳手術歴（history of ear surgery）history of ear surgery（耳手術歴）、感染、単一聴耳では洗浄を避け、専門的除去を考える

外骨腫

冷水曝露に関連する外耳道骨性隆起で、通常は両側・多発・広基性wide-based (broad-based) 広基性（wide-based (broad-based)）wide-based (broad-based)（広基性） である。小さければ経過観察し、外耳道閉塞、反復外耳炎recurrent otitis externa反復外耳炎（recurrent otitis externa）recurrent otitis externa（反復外耳炎）、耳垢貯留cerumen impaction耳垢貯留（cerumen impaction）cerumen impaction（耳垢貯留）、伝音難聴conductive hearing loss 伝音難聴（conductive hearing loss）conductive hearing loss（伝音難聴） （CHL）があれば外耳道形成 external auditory canal reconstruction 外耳道形成（external auditory canal reconstruction） external auditory canal reconstruction（外耳道形成） 術を検討する。骨腫は単発・有茎性pedunculated 有茎性（pedunculated）pedunculated（有茎性） が多い。

耳介の炎症

軟骨膜炎Perichondritis 軟骨膜炎（Perichondritis）Perichondritis（軟骨膜炎） では緑膿菌を含む抗菌薬選択と、膿瘍があれば排膿が必要である。

耳性帯状疱疹

潜伏水痘・帯状疱疹ウイルス varicella-zoster virus 水痘・帯状疱疹ウイルス（varicella-zoster virus） varicella-zoster virus（水痘・帯状疱疹ウイルス） が膝神経節 geniculate ganglion 膝神経節（geniculate ganglion） geniculate ganglion（膝神経節） 周辺で再活性化し、耳痛otalgia 耳痛（otalgia）otalgia（耳痛） に続いて耳介・外耳道・口腔の水疱、末梢性顔面神経麻痺peripheral facial nerve palsy 末梢性顔面神経麻痺（peripheral facial nerve palsy）peripheral facial nerve palsy（末梢性顔面神経麻痺） を生じる。感音難聴、回転性めまい（vertigo）、眼振、頸部リンパ節腫脹cervical lymphadenopathy 頸部リンパ節腫脹（cervical lymphadenopathy）cervical lymphadenopathy（頸部リンパ節腫脹） を伴い得る。

• 診断：典型的皮疹と顔面神経麻痺 facial nerve palsy顔面神経麻痺（facial nerve palsy） facial nerve palsy（顔面神経麻痺）。皮疹が遅れる、または無疹性帯状疱疹 zoster sine herpete 無疹性帯状疱疹（zoster sine herpete） zoster sine herpete（無疹性帯状疱疹） もある
• 治療：早期全身性抗ウイルス薬 early systemic antiviral 早期全身性抗ウイルス薬（early systemic antiviral） early systemic antiviral（早期全身性抗ウイルス薬） と副腎皮質ステロイドを患者背景に応じて併用し、眼球保護eye protection 眼球保護（eye protection）eye protection（眼球保護） を徹底する

外耳炎

外耳道皮膚の急性びまん性炎症で、主に緑膿菌と黄色ブドウ球菌が関与する。

リスク因子

• 水泳・湿潤、綿棒cotton swab 綿棒（cotton swab）cotton swab（綿棒） や掻破 scratching (excoriation) 掻破（scratching (excoriation)） scratching (excoriation)（掻破） による微小外傷 microtrauma微小外傷（microtrauma） microtrauma（微小外傷）

所見・診断

耳痛otalgia耳痛（otalgia）otalgia（耳痛）、掻痒、耳閉感aural fullness耳閉感（aural fullness）aural fullness（耳閉感）、耳漏otorrhea耳漏（otorrhea）otorrhea（耳漏）、軽度伝音難聴 conductive hearing loss 伝音難聴（conductive hearing loss） conductive hearing loss（伝音難聴） を呈し、耳珠圧迫tragal pressure 耳珠圧迫（tragal pressure）tragal pressure（耳珠圧迫） または耳介牽引 auricular traction 耳介牽引（auricular traction） auricular traction（耳介牽引） で疼痛が増悪する。外耳道皮膚は発赤・浮腫し、デブリdebris デブリ（debris）debris（デブリ） や膿性分泌物 purulent discharge 膿性分泌物（purulent discharge） purulent discharge（膿性分泌物） を伴う。鼓膜は保たれるが、腫脹で見えないことがある。

鼓膜穿孔tympanic membrane perforation 鼓膜穿孔（tympanic membrane perforation）tympanic membrane perforation（鼓膜穿孔） が疑われる場合は耳毒性の（ototoxic）可能性のあるアミノグリコシド系薬 Aminoglycosides アミノグリコシド系薬（Aminoglycosides） Aminoglycosides（アミノグリコシド系薬） 滴下 dribbling 滴下（dribbling） dribbling（滴下） を避け、非耳毒性製剤を選ぶ。単純外耳炎 otitis externa 外耳炎（otitis externa） otitis externa（外耳炎） （OE）に全身性抗菌薬は原則不要だが、耳介周囲蜂窩織炎periauricular cellulitis耳介周囲蜂窩織炎（periauricular cellulitis）periauricular cellulitis（耳介周囲蜂窩織炎）、免疫不全、感染拡大では適応となる。

壊死性外耳炎

外耳道感染が側頭骨 temporal bone側頭骨（temporal bone） temporal bone（側頭骨）・頭蓋底へ進展する頭蓋底骨髄炎で、高齢者、糖尿病、免疫不全に多い。緑膿菌が代表的だが、他の細菌、真菌、培養陰性例もある。¹

⚠️ 高齢者、糖尿病、免疫不全患者で、夜間に強い治療抵抗性耳痛 otalgia耳痛（otalgia） otalgia（耳痛）、持続する膿性耳漏 otorrhea耳漏（otorrhea） otorrhea（耳漏）、外耳道床floor of the external auditory canal 外耳道床（floor of the external auditory canal）floor of the external auditory canal（外耳道床） の肉芽 granulation肉芽（granulation） granulation（肉芽）、脳神経障害cranial neuropathy 脳神経障害（cranial neuropathy）cranial neuropathy（脳神経障害） を認めたら疑う。

• 培養検査culture培養検査（culture）culture（培養検査）、炎症性マーカー、高分解能high resolution, HR 高分解能（high resolution, HR）high resolution, HR（高分解能） コンピュータ断層撮影（CTCT, computed tomography）と造影磁気共鳴画像 contrast-enhanced MRI 造影磁気共鳴画像（contrast-enhanced MRI） contrast-enhanced MRI（造影磁気共鳴画像） を相補的に用い、骨・軟部組織・頭蓋内進展intracranial extension 頭蓋内進展（intracranial extension）intracranial extension（頭蓋内進展） を評価する。治療抵抗性病変では悪性腫瘍との鑑別に生検を行う
• 入院を含む専門管理、培養結果blood/culture results 培養結果（blood/culture results）blood/culture results（培養結果） に基づく長期全身性抗微生物薬治療 antimicrobial treatment抗微生物薬治療（antimicrobial treatment） antimicrobial treatment（抗微生物薬治療）、糖尿病管理、必要時デブリードマン debridementデブリードマン（debridement） debridement（デブリードマン）。結果判明までは緑膿菌を含む経験的治療 empiric therapy 経験的治療（empiric therapy） empiric therapy（経験的治療） を検討する
• 高圧酸素療法hyperbaric oxygen therapy 高圧酸素療法（hyperbaric oxygen therapy）hyperbaric oxygen therapy（高圧酸素療法） は標準治療を置換せず、難治例で補助的に検討される

外耳腫瘍

外耳道悪性腫瘍は外耳炎 otitis externa 外耳炎（otitis externa） otitis externa（外耳炎） に似た非特異的症状で診断が遅れ得る。治らない潰瘍・出血・疼痛・耳漏otorrhea耳漏（otorrhea）otorrhea（耳漏）・顔面神経症状では生検と画像評価を行い、組織型と病期に応じて治療する。²

まとめ

• 耳珠圧痛tragal tenderness耳珠圧痛（tragal tenderness）tragal tenderness（耳珠圧痛）・外耳道浮腫・正常鼓膜normal tympanic membrane 正常鼓膜（normal tympanic membrane）normal tympanic membrane（正常鼓膜） は外耳炎 otitis externa 外耳炎（otitis externa） otitis externa（外耳炎） の典型である。
• 耳介血腫auricular hematoma 耳介血腫（auricular hematoma）auricular hematoma（耳介血腫） は早期排膿 early drainage 早期排膿（early drainage） early drainage（早期排膿） と圧迫で軟骨変形を防ぐ。
• 糖尿病患者の強い夜間耳痛 otalgia耳痛（otalgia） otalgia（耳痛）・肉芽granulation肉芽（granulation）granulation（肉芽）・脳神経障害cranial neuropathy 脳神経障害（cranial neuropathy）cranial neuropathy（脳神経障害） は壊死性外耳炎 necrotizing otitis externa 壊死性外耳炎（necrotizing otitis externa） necrotizing otitis externa（壊死性外耳炎） （OE）を疑う。
