🧠 全体像:どのトランスポーターによる再取り込みを阻害するかで分けるのがコツ。 5-HTのみ=SSRI/5-HT+NE=SNRISNRI(serotonin-noradrenaline reuptake inhibitor)・TCA/NEのみ=NRI/NE+DA=NDRI(ブプロピオンbupropionブプロピオン(bupropion)bupropion(ブプロピオン))。 語尾ヒントとして -oxetine系は多様(フルオキセチンfluoxetine フルオキセチン(fluoxetine)fluoxetine(フルオキセチン) =SSRISSRI(selective serotonin reuptake inhibitor)、デュロキセチンduloxetine デュロキセチン(duloxetine)duloxetine(デュロキセチン) =SNRI、レボキセチンreboxetineレボキセチン(reboxetine)reboxetine(レボキセチン)・アトモキセチンatomoxetine アトモキセチン(atomoxetine)atomoxetine(アトモキセチン) =NRI)なので、語尾ではなく機序で覚える。

効果と機序をめぐる2つの論争
このクラスは「よく効く薬」として教わるが、効果量と機序のどちらにも決着していない議論がある。試験では標準的な説明を答えつつ、下の位置づけを知っておく。
💡 効果量の論争。 プラセボplacebo プラセボ(placebo)placebo(プラセボ) との差はうつ病の初期重症度が軽いほど小さく、中等度以下ではほとんど差がないという指摘がある一方1、522試験・116,477人を統合したネットワークメタ解析 network meta-analysis ネットワークメタ解析(network meta-analysis) network meta-analysis(ネットワークメタ解析) では21剤すべてがプラセボ placebo プラセボ(placebo) placebo(プラセボ) より有意に有効(オッズ比odds ratio, OR オッズ比(odds ratio, OR)odds ratio, OR(オッズ比) 1.37〜2.13)で、薬剤間の差は比較的小さいと報告されている2。⚠️ 後者の著者自身がエビデンスの確実性を中等度〜非常に低いと評価している点まで含めて覚える。
💡 「うつ病=セロトニン不足」という仮説の論争。 主要な研究領域を統合したアンブレラレビューは、抑うつがセロトニン濃度・活動の低下で引き起こされるという仮説を支持する一貫した証拠は無いとした3。一方、トリプトファンtryptophan トリプトファン(tryptophan)tryptophan(トリプトファン) 枯渇試験・神経内分泌学的所見・PETPET(positron emission tomography)研究などを根拠に、前シナプスpresynaptic 前シナプス(presynaptic)presynaptic(前シナプス) のセロトニン神経活動の低下を示す証拠は残っているとする総説もあり、後者はアンブレラレビューの結論 conclusion 結論(conclusion) conclusion(結論) には議論の余地があるとしている4。両者が一致しているのは「単一の神経伝達物質の欠損でうつ病を説明するのは妥当でない」という点である。
⚠️ 「効く機序が分かっていない」は「効かない」ではない。 機序の仮説と臨床効果の証拠は別の話題なので、2つの論争を混ぜない。
SSRI(選択的セロトニン再取り込み阻害薬)
第一選択。セロトニントランスポーターserotonin transporter (SERT) セロトニントランスポーター(serotonin transporter (SERT))serotonin transporter (SERT)(セロトニントランスポーター) (SERT)だけを阻害するので、TCAの抗コリン作用 anticholinergic effects抗コリン作用(anticholinergic effects) anticholinergic effects(抗コリン作用)・心毒性を持たない——これが「安全になった」と言われる中身である。

SNRI(セロトニン・ノルアドレナリン再取り込み阻害薬)
5-HTに加えてNEも阻害する。下行性疼痛抑制系descending pain inhibitory system下行性疼痛抑制系(descending pain inhibitory system)descending pain inhibitory system(下行性疼痛抑制系)を介して痛みにも効くのがSSRIとの決定的な違いである。

⚠️ 心毒性は「過量時の話」だけではない。禁忌の欄にすでに書かれている。 英国のアミトリプチリン amitriptyline アミトリプチリン(amitriptyline) amitriptyline(アミトリプチリン) SmPCは最近の心筋梗塞とあらゆる程度の心ブロック heart block心ブロック(heart block) heart block(心ブロック)・調律rhythm 調律(rhythm)rhythm(調律) 異常・冠動脈不全を禁忌に挙げている(ほかに重症肝疾患、6歳未満、MAO阻害薬の併用)10。「心疾患があるから慎重に」ではなく、該当すれば使わない規定である。
⚠️ 過量摂取overdose 過量摂取(overdose)overdose(過量摂取) の心電図所見と、危険を判定する物差し。 同じSmPCは、過量時の心電図がPR間隔PR interval PR間隔(PR interval)PR interval(PR間隔) の延長・QRS幅の拡大・QT延長・T波の平低化/陰転・ST低下ST depressionST低下(ST depression)ST depression(ST低下)・各種の心ブロック heart block心ブロック(heart block) heart block(心ブロック)を経て心停止へ進みうるとし、「QRS幅の拡大は急性過量後の毒性の重症度とよく相関する」と明記する。実務上の目安は QRS > 0.16秒 を探すこと、心機能の連続心電図監視を3〜5日間続けること、成人では 750 mg以上の摂取で重篤な中毒がありうること。市販後にはブルガダ症候群の顕在化・ブルガダ型心電図の報告もある10。
⚠️ 混合中毒でフルマゼニル flumazenil フルマゼニル(flumazenil) flumazenil(フルマゼニル) を使わない。 同じSmPCの管理手順は逐語で「混合過量摂取 overdose 過量摂取(overdose) overdose(過量摂取) においてベンゾジアゼピン毒性を拮抗する目的でフルマゼニル flumazenil フルマゼニル(flumazenil) flumazenil(フルマゼニル) を投与しないこと」と指示している10。TCAが同時にあると、けいれん閾値seizure threshold けいれん閾値(seizure threshold)seizure threshold(けいれん閾値) を下げる方向に働くためである。
💡 MAO阻害薬との切り替え間隔は、相手が不可逆的か可逆的かで変わる。 アミトリプチリンamitriptyline アミトリプチリン(amitriptyline)amitriptyline(アミトリプチリン) は、不可逆的な非選択的MAO阻害薬の中止から14日後、可逆的なモクロベミド moclobemide モクロベミド(moclobemide) moclobemide(モクロベミド) の中止からは最短1日後に開始できる。逆にアミトリプチリン amitriptyline アミトリプチリン(amitriptyline) amitriptyline(アミトリプチリン) を止めてMAO阻害薬を始めるときは14日あける10。
